Here are the following requirements an applicant must submit:

Completed Application Form

Proof of Clearance (Background check result/Live Scan Result)

Home Care Aide (HCA) number

State required training: 5hrs- entry level training; 5hrs – annual training

Valid Government Issued ID/ Driver’s License

Valid SSN or ITIN (for business only)

CPR/AED Certified (valid for 2 years)

TB Test or Chest X-RAY Result (valid for 2 years)

Flu shot, Covid shot vaccine (valid for 2 years)
